All school form requests must include our completed School Form Request. This form must be fully completed and emailed to:

All forms must be submitted in PDF format. We are not able to process forms submitted in other formats, and we are not able to download forms provided through a link.
Once a complete request is received, our office will review the documentation and send an electronic invoice for the associated fee. Form processing begins once payment has been received.
Please note: If the patient has not had a visit with their physician or PA within the last year, an appointment will be required before forms can be completed. Challenges, Day 1 appointments, and OIT updose visits are procedures and are not considered provider visits for the purpose of form completion.
